CMS-2: Preventative Care and Screening: Screening for Depression and Follow-up Plan. We are seeking clarification on if a permanent cognitive impairment can be used as a denominator exception permanently or if it would just be for the reporting year? If a medical reason of cognitive impairment is documented during a visit in 2025 as a reason for not screening the patient for depression and it's deemed to be a permanent impairment, would we need to also document that medical reason each year after that to be compliant and have this patient be an exception?
