CMS-2: clarification on permanent cognitive impairment

      Thank you for inquiring about the Preventive Care and Screening: Screening for Depression and Follow-Up Plan eCQM. Please note that the medical reason for not screening the patient for depression must be documented during a relevant qualifying encounter each year to count as an exception. An existing or prior diagnosis of cognitive impairment will not automatically be captured as an exception. During each measurement period, a code from the Medical Reason value set must be documented during a relevant qualifying encounter as a reason for not screening for depression in order for the patient to be considered an exception.       CMS-2: Preventative Care and Screening: Screening for Depression and Follow-up Plan. We are seeking clarification on if a permanent cognitive impairment can be used as a denominator exception permanently or if it would just be for the reporting year? If a medical reason of cognitive impairment is documented during a visit in 2025 as a reason for not screening the patient for depression and it's deemed to be a permanent impairment, would we need to also document that medical reason each year after that to be compliant and have this patient be an exception?
