      Thank you for your inquiry regarding CMS72/STK5. As stated in the measure rationale: “anticoagulants at doses to prevent venous thromboembolism are insufficient antithrombotic therapy to prevent recurrent ischemic stroke or transient ischemic attacks (TIA).” This is also further specified in the exclusion criteria of the the value set "Antithrombotic Therapy for Ischemic Stroke." As such, 5000 units subq of Heparin is not appropriate for stroke but is appropriate for Venous Thromboembolism prophylaxis. ​​
