Multiple HbA1c on same date, one with NULL value

      Thank you for your inquiry about CMS122v13 (2025 Performance Period). A patient with a HbA1c with a null value will still meet numerator criteria. In scenario 1, if a patient had multiple hbA1c completed on the same day, a null value will be the lowest assessment reading pulled into the definition "Has Most Recent Glycemic Status Assessment Without Result" to meet numerator criteria. In scenario 2, the hbA1c reading with a result of 8% will be the lowest result considered and not meet the numerator criteria. Please see the logic below.

      CMS 122 - Glycemic Status Control >9% specifications state that if multiple glycemic status assessments are recorded on the same date, use the lowest result.

      The measure logic appears to use the Most Recent glycemic status assessment.

      SCENARIO 1:  Patient meets criteria to be included in the measure population.  

      Patient has multiple HbA1c's on the same date.  

      • HbA1c on 2/26/2025 @ 9:22 am with a result of 8%
      • HbA1c on 2/26/2025 @ 9:22am with a result of NULL

      Would the patient be considered as in control or not in control for the measure?

       

      SCENARIO 2: 

      Patient meets criteria to be included in the measure population.  

      Patient has multiple HbA1c's on the same date.  

      • HbA1c on 2/26/2025 @ 9:22 am with a result of 8%
      • GMI on 2/26/2025 @ 9:22am with a result of 10%

      Would the patient be considered as in control or not in control for the measure?
