Thank you for inquiring about the Preventive Care and Screening: Screening for Depression and Follow-Up Plan eCQM. The measure’s Guidance section indicates “This measure does not require documentation of a specific score, just whether results of the normalized and validated depression screening tool used are considered positive or negative.” Some sites may retain provider-facing documentation as scores, and then map that information to the measure’s relevant codes for positive or negative:
      · SNOMEDCT Code 428171000124102: Depression screening negative (finding)
      · SNOMEDCT Code 428181000124104: Depression screening positive (finding)

Your implementation should reflect your organizational workflows and vendor reporting, as the measure does not impose limitations: either the measure’s coded elements can be explicitly referenced within the medical record (positive or negative finding), or they can be mapped for reporting (medical record retains a score and that is mapped to positive or negative finding for reporting with vendor support). You should consult with your measurement reporting vendor before making any changes to your documentation.

    • Seeking clarification on requirement of positive/negative result from depression screening.

CMS-2 Preventative Care and Screening; Screening for Depression and Follow-up Plan. We are seeking clarification on the requirement of the screening result of positive/negative. We are using a validated tool and the score on the tool dictates the associated score. Do we need documentation of positive/negative or does the score suffice?
