    • The codes that CMS includes in this measure each year do not always match what the official coding guidelines are for the same time period, so we struggle to accurately capture appropriate codes

      I'd like to suggest that CMS make more of an effort to coordinate the timing and code selection for this measure with the Official Coding Guideline releases. This measure uses so many codes and for the last two years we have had issues with coding guidelines changing that required our coding department to use new codes that CMS hasn't factored in to the measure. To give just one example, the coding guidelines around morbid obesity and class III obesity changed on October 1st of 2024, so unless providers used the term "morbid obesity" on their H&P, we couldn't code for morbid obesity and have it factor into our risk adjustment. This is an outdated and offensive term, but we got the providers on board to use it in their H&Ps on admission. Then on April 1st, they updated the Official coding guidelines so that if Class III obesity is anywhere in the documentation, they need to choose that code, not the morbid obesity code we had educated the providers to use in the H&P. This makes it even more difficult to get a code that captures that risk factor accurately in our population. The new codes for PC-07 in 2026 will finally correct this issue. The Official Coding Guidelines typically go into effect for hospitals on April 1 and October 1, so it would be nice if CMS tried to coordinate their updated measure specs with one of these times.
