If documentation of discharge medications is done in multiple places, your organization may need to work with staff on documentation in discrete data fields or work with your IT staff or EHR vendor to identify the fields where this documentation is occurring.

      Is there a way to pick up a free text addendum in the discharge summary, stating the patient was discharged on an antithrombotic to avoid a mismatch if validated? If not, how does ECQM measures meet the requirements if there is an addendum in the chart?
