      Thank you for your question regarding CMS71v14 and CMS71v15, Anticoagulation Therapy for Atrial Fibrillation/Flutter. The measure uses the data type "Medication, Not Discharged" to identify denominator exceptions based on the "Medical Reason For Not Providing Treatment" and "Patient Refusal" value sets. The "Medication, Discharge" data type looks for the author dateTime of the discharge medication reconciliation form authored during the inpatient encounter ("Timing: The time the discharge medication list on the discharge instruction form is authored”). The timestamp must occur during the encounter, and not after the encounter ends.

      Can the physician add a reason for contraindication for no anticoagulation at discharge after it has been final abstracted, as long as it has been added within 30 days of discharge. Per validation guidelines ECQM charts will be validated same way as abstracted charts. would this include within 30-days of discharge?
