Patients qualifying for the numerator must have a coded diagnosis mapped to the "Severe Maternal Morbidity Diagnoses" value set (2.16.840.1.113762.1.4.1029.255) and coded with that diagnosis as "Present on Admission is No or Unable to Determine" (2.16.840.1.113762.1.4.1029.370). Present on Admission (POA) is most often captured in claims data where diagnoses are coded using ICD-10. However, some electronic health records (EHR) platforms can capture POA within documentation like the Problem List where SNOMED is often used. We recommend that you confirm with your EHR vendor and/or IT department.

      Should Diagnosis from a problem list that are not confirmed qualify patient for numerator?

      Should it be from confirmed final Dx only? our EHR is using Problem list that are SNOMED codes unconfirmed. is this correct? or should they be pulling only confirmed SNOMED or ICD10 codes that are final?
