Seeking clarification to ensure our facility is capturing accurate data by completing CQM form correctly.
In our EHR, with have a CQM documentation section that our labor and delivery department staff completes during patient hospitalization. I would like clarification on how to fill this form out appropriately. We complete this form based on our EHR company instructed us that this documentation is required to capture our data correctly.
1. Do we complete this form on every labor and delivery patient that is admitted?
2. How is the form filled out based on different scenarios?
The form I am referring to includes these questions The predelivery questions that includes: estimated gestational age, estimated delivery date, number of pregnancies, number of preterm births, number of full-term births, parity, fetus presentation with options for fetus A, B, C, D. The delivery question of- date and time of obstetric delivery.
*Scenario 1, a patient is admitted with first time pregnancy outcome is a miscarriage at 18 weeks gestation.
3. Would our staff complete this CQM documentation for this patient?
4. If yes, for the predelivery questions would they input number of pregnancies 1, number of preterm births 1, number of full term births 0, parity 1? Or would parity be 0 as this was not a live birth?
5. For the fetus presentation section- are these for mothers who are pregnant with twins? Triples, etc?
