    • Workflow considerations related to data collection

      During the post-op data collection window for each patient, is there a requirement that someone from the clinical team (either the physician or the performing facility) is required to collect the post-op HOOS Jr. or KOOS Jr. directly or would we be able to leverage a paper-survey, mailed to the patient and returned to the system, to then be transcribed into the record?  

      This is of particular interest in understanding the way in which we are able to collect the information from the patient, in the event they are not responsive to the follow-up calls from the system or the provider.
