Thank you for your inquiry about CMS128v14 (2026 Performance Period). Patients who have received antidepressant medication for at least 180 days of continuous treatment during the 231-day period following the Index Prescription Start Date would meet Numerator 2. The measure's CQL logic requires documentation of a medication dispensing event using the QDM datatype "Medication, Dispensed" along with the value set "Antidepressant Medication". Continuous treatment is defined by the duration of medication dispensing events calculated based on data attributes such as days supplied. Therefore, having a documentation of a medication prescription with refills alone would not meet requirements. Please review the measure specification for further details about measure requirements: https://ecqi.healthit.gov/sites/default/files/ecqm/measures/CMS128-v14.0.000-QDM.html.

To meet CMS measure 128: Anti-depressant Medication Management, do patients need to have their medications dispensed from a pharmacy? Is a provider’s 90-day prescription with three refills alone insufficient to meet numerator 2 of this measure?
