      Thank you for your inquiry regarding CMS108v12, Venous Thromboembolism Prophylaxis (VTE-1). Continuous heparin drip does not qualify the patient for the VTE prophylaxis measures. The Guidance section in the measure specification states, “when low dose unfractionated heparin is administered for VTE prophylaxis, the intended administration route for low dose unfractionated heparin is subcutaneous.” One-time IVP doses of heparin are not sufficient for VTE prophylaxis. It is therefore necessary to provide a reason for not administering VTE prophylaxis for the patient to qualify for the VTE-1 numerator.

    • Numerator Criteria

      Patient is in the initial population and meets denominator criteria. 

      For VTE prophylaxis, if a patient is on a continuous heparin drip from the day of admission through at least the day after, do you need further documentation of a reason for no VTE prophylaxis in order to be counted in the numerator?

      Example: Admission Date: 12/30/20XX

      In the MAR: Heparin 100 units/ml in 5% Dextrose Freq. Continuous: Start: 12/30/20XX and End 1/6/20XX
