    • All listed therapy encounters aren't appropriate to include as eligible encounters and creates inaccurate performance rates.

      Including various therapy encounters in the value set for this measure that do not align with actual clinical practice creates a flawed performance rate. Examples of encounters where medication reconciliation is not expected to occur or is out of scope for the professional conducting the encounter are listed below. We recommend the measure steward reevaluate all codes associated with auditory, physical, occupational, psycho, speech and other therapies and utilize heightened discernment based on real world clinical practice.
