      For the Preventive Care and Screening: Screening for Depression and Follow-Up Plan measure, within the 'Guidance' section of the measure specification, it states "This measure does not require documentation of a specific score, just whether results of the normalized and validated depression screening tool used are considered positive or negative. Each standardized screening tool provides guidance on whether a particular score is considered positive for depression." So, after the screening is completed and the score is calculated, is it necessary to explicitly document "positive screening" or "negative screening," or is listing the interpretation of the screening tool used sufficient? For example, does the interpretation satisfy the need for indicating a positive result, or should we also include a notation within our documentation explicitly stating, "positive screening"?
