      Thank you for inquiring about the Preventive Care and Screening: Screening for Depression and Follow-Up Plan eCQM. Please note that the measure is not prescriptive as to which tools may be used to screen for depression, but rather only requires that providers use “A normalized and validated depression screening tool developed for the patient population in which it is being utilized” (eCQM Definition section). The list contained within the v14 specification is not all-encompassing. Therefore, if the PHQ-2 or another tool is normalized and validated with clear guidance on whether a particular score is considered positive for depression, it may be used to evaluate the measure’s performance. If the score is positive for even mild depression based on the thresholds established by the normalized and validated depression screening tool, then a follow-up plan is required. The follow-up plan must include interventions that represent emotional and coping support as well as mental health management to follow up on previously evaluated and diagnosed depression or depressive disorder in the patient population treated, and not just reassessment using a different depression screening tool.
            To directly respond to your questions:
      a. Additional screening is not required if a normalized and validated tool determines that a patient has even mild depression.
      b. If a patient meets all other qualifying criteria (and no documented exclusion), screens positive for depression using a normalized and validated tool, and has a timely documented follow-up plan, then they qualify for the numerator.

      If a patient is screened for depression using the PRIME MD PHQ-2 as the screening tool, and the screening is positive, is any additional screening required, such as completion of a PHQ-9, in order to meet the numerator criteria? Or is using the PHQ-2 alone sufficient, and as long as a positive interpretation and appropriate follow-up is documented, then the numerator criteria will be met?
