Thank you for your question regarding CMS871v4, Hospital Harm - Severe Hyperglycemia (HH-Hyper). You are correct that the measure calculation is different than CMS816v4, Hospital Harm - Severe Hypoglycemia (HH-Hypo), because HH-Hyper assesses the number of days during an encounter in which a hyperglycemic event occurs. Specifically, the measure assesses the number of inpatient hospital days for patients age 18 and older with a hyperglycemic event (harm) per the total qualifying inpatient hospital days for that encounter. This measure defines a hyperglycemic event as either a day with at least one glucose value >300 mg/dL OR a day where a glucose test and result was not found, and it was immediately preceded by two contiguous, consecutive days where at least one glucose value during each of the two days was >=200 mg/dL.

I am still attempting to understand this measure HH-02. Can you please clarify for me that it is different than HH-01 which only takes the "first" episode of hypoglycemia? HH-02 "penalizes" us for each day in which a hyperglycemic event occurs—is that correct? Thank you.
