Thank you for inquiring about the Preventive Care and Screening: Screening for Depression and Follow-Up Plan eCQM. Please note that only structured documentation can be used to evaluate eCQM performance; therefore, if the documentation reference is unstructured notes indicating a verbal referral to the PCP was done, then this documentation alone would not suffice. The measure includes three ways to meet the follow-up component: "Medication, Order," "Intervention, Order" (referral), and "Intervention, Performed" (which contains a multitude of options to meet follow-up). For example, if the verbal referral was documented in the record using SMOMED-CT code 703978000 – “Referral to primary care service (procedure)” in a structured field, this could meet the “Intervention, Ordered” criterion. You may need to engage with your vendor to verify from which structured documentation location referral information is retrieved (this may be clinical orders or another location) for reporting.

Will documentation of a verbal discussion indicating that the specialist provider referred the patient to their primary care provider to discuss evaluation for depression satisfy the Follow-Up Plan? Is an electronic referral order also required or will the aforementioned documentation suffice?
