      For the eCQM VTE measure, our facility has a physician order to map too the QRDA file of they enter an order for No VTE due to low risk. Sometimes the physician does not enter that order but they do put it in the body of a note that the mapping does not capture.

      My questions is- If we discover this in a chart can we retrospectively go back into that chart and enter the order for the QRDA mapping to pick up if we can see that it is in the physicians note?
