      PC-07 was developed and modeled based on the Centers for Disease Control and Prevention (CDC)’s definition but the terminology for each Severe Maternal Morbidity (SMM) condition and procedure should be mapped from your electronic health record (EHR) to the value sets found within the Terminology section of the measure specification. The value sets and applicable codes for this measure can be viewed in the VSAC (Value Set Authority Center) located at https://vsac.nlm.nih.gov/.

      In the example given, in which a patient receives a blood transfusion during the delivery encounter, documentation of blood transfusion administration is typically found within the Flowsheets activity, specifically in the Blood Transfusion or Blood Administration flowsheet. If you find that your facility does not include blood transfusion on claims, mapping to the flowsheet would be ideal. If the blood transfusion is the only Severe Obstetric Complication, the delivery encounter will be removed from the numerator.

      Our hospital is working toward building a query to report PC-07.  Is this measure specific to SMM coding for inclusion? I realize there are exclusions, but once those are applied, does the patient have to have the SMM diagnosis and/or procedure codes provided by the CDC to be reported in the numerator?

      Example, a patient receives a blood transfusion during the delivery encounter but does not have a procedure code for the transfusion. Should we include her in the numerator?
