Thank you for your inquiry regarding CMS816v3: Hospital Harm – Severe Hypoglycemia. The measure assesses the number of inpatient hospitalizations for patients age 18 and older who were administered at least one hypoglycemic medication during the encounter, who suffer the harm of a severe hypoglycemic event during the encounter. Patients being treated for hyperkalemia are not included in the denominator for this measure unless they also receive a hypoglycemic medication during the encounter. This measure does not contain any exclusions. To meet the numerator criteria of a severe hypoglycemic event, there must be evidence of a glucose test with result less than 40 mg/dL where a hypoglycemic medication was administered within the 24 hours before the start of the severe hypoglycemic event.
