      Thank you for your inquiry regarding CMS165v13 (2025 Performance Period). To answer your first question, the logic is specified such that if the providers are sharing EHRs or sharing data through Health Information Exchange, all providers that have access to the EHR documentation or data could use for reporting. A patient will not be eligible for the measure if there is no documented evidence of an active essential hypertension diagnosis using a code from the "Essential Hypertension" value set (2.16.840.1.113883.3.464.1003.104.12.1011) within the first six months of the measurement period.

      To answer your second question, the measure logic specifies that diagnoses such as essential hypertension must be active within the first six months of the measurement period to qualify for initial population, as expressed in the CQL logic below. If the diagnosis is resolved before the start of the measurement period, it does not meet the criteria.

      For the eCQM blood pressure measure, the numerator is defined as: 
      Patients 18-85 years of age by the end of the measurement period who had a visit during the measurement period and diagnosis of essential hypertension starting before and continuing into, or starting during the first six months of the measurement period.
       
      I have two questions, please: 
      1. If the patient's diagnosis of hypertension was established by a provider who is not a member of our ACO and - as a result - the diagnosis is not present in the EHR used by providers in the ACO, would the patient qualify for the denominator?
      2. Where can I find guidance on what it means to have a diagnosis that is "continuing into" a measurement period?
