Numerator Definition for CMS 68 -Documentation of Medications in the Medical Record

      Thank you for inquiring about the Documentation of Current Medications in the Medical Record eCQM. The measure intends to capture all encounters where the current medications taken by the patient are reconciled/reviewed (e.g., new added, old removed, active noted as reviewed). This requires an explicit timestamp during the encounter relevant period. You may need to work with your EHR vendor to ensure that data are retrieved from the correct structured location for documenting medication reconciliation (i.e., documentation in a free text summary is not considered for an eCQM). The Guidance section of the specification indicates “This eCQM is an episode-based measure. An episode is defined as each eligible encounter during the measurement period. This measure is to be reported for every eligible encounter during the measurement period.” Therefore, please report reconciliation for every qualifying encounter during the measurement period. Additionally, the measure is not prescriptive as to whom completes the reconciliation, only that it is done. Please let us know if you require further clarification.

      Dear Support,

      Could you please help clarify the numerator definition of this QI 2 care coordination measure (CMS 68 -Documentation of Medications in the Medical Record)? Does it mean that as long as there is documentation of medications in the medical record, it meet the numerator criteria? Or, does it mean that the medications need to be reviewed/reconciled at every encounter? If yes, does the review have to be done by a provider, or can the medications be reviewed by a pharmacist or nursing staff?
