Thank you for your inquiry regarding CMS122v13 (2025 Performance Period). Documentation is needed for "Glucose Management Indicator" LOINC code (97506-0) using QDM datatype "Laboratory Test, Performed", per measure intent and specification. According to QDM 5.6, "Laboratory Test represents a medical procedure that involves testing a sample of blood, urine, or other substance from the body. Tests can help determine a diagnosis, plan treatment, check to see if treatment is working, or monitor the disease over time." If you need assistance with implementing this specification, please refer to guidance on eCQI resource center, such as https://ecqi.healthit.gov/ecqm-implementation-checklist.

We are wondering why the Glucose Management Indicator LOINC was classified under "Laboratory Test, Performed" in CMS122v13 of the Diabetes: Glycemic Status Assessment Greater Than 9% measure? The GMI is produced by a patients continuous glucose monitor and is taken while the patient is seen during clinic visits and is not an ordered laboratory test that is performed by a laboratory. We are struggling to map this data point within our EMR as it can just be entered as a discreet data point in what is known as a flowsheet but our EMR cannot map "labs" to flowsheets.

Examples identified on the eCQI website under "Laboratory Test, Performed": "Glucose management indicator" include:

      • Initiation of a venipuncture for a fasting blood glucose to the time venipuncture for the fasting blood glucose is completed – use relevant dateTime to reference this single point in time for many specimen collections
      • Initiation of a 24-hour urine collection for measured creatinine clearance until completion of the 24-hour urine collection – use relevantPeriod to reference the start to stop time for this event.

Obtaining a GMI value does not involve the collection of any bodily fluids and is not a laboratory test that can be ordered by a clinician. It is a calculation based of the variable of mean glucose values over a determined period of time where the mean glucose value is obtained by the continuous glucose monitor. Is there a different Data Element category that this LOINC can be assigned to?
