      If your electronic medical record (EMR) documentation aligns with these criteria and is mapped correctly, you would abstract this as MO1=1 ("Not at Risk").       Our EMR has an Adult Patient History with section titled "Nutrition" and under that section it states "Nurtitional Risk Factors: Criteria Not Met/No Consult Needed" which is performed/documented by Nurse.  No order for Nutrition Consult/Dietitian.  According to Specifications, I would abstract this as MO1=1 "Not at Risk" and measure calculation would stop at MO1 with MO2-MO4=N/A with calculation of 1/1 X 100=100%.  Based on the documentation by nurse in Patient History, is this correct?
