      1. There is no code that specifically fits this exception as laid out here. The measure specifications allow for tools, such as the Short Informant Questionnaire on Cognitive Decline in the Elderly, that can be used by an informant rather than the patient.
      2. This measure may not be a good fit for reporting purposes for these types of clinicians. The initial population of this measure captures those with dementia who have had two or more visits during the measurement period. This requirement is to establish that the eligible clinician has an existing relationship with the patient and is caring for their dementia.

      Are there exceptions for the following scenarios:

      1. Patients that are unable to complete the assessment due to the severity of their dementia status.
      2. Patients that follow with another physician (i.e. Neurologist) for dementia.  Where our physician doesn't actually administer the test, but is involved in the patients care.
