According to the measure logic, if emergency department (ED) end datetime is the same as observation start datetime and inpatient start datetime is the same as observation end datetime, then the start of inpatient hospitalization is referring to the ED start datetime.

      The measure would follow the process as outlined below –
      1. The start of the hospitalization is ED start datetime (01/21/2025) since ED ends within 1 hour from Observation start datetime, and Observation ends the same time as inpatient admission start datetime.
      2. The inpatient admission is on 01/23/2025 and IPC applied 01/24/2025, which is one day after the inpatient admission.
      3. The case satisfies the numerator condition included in the “Encounter With VTE Prophylaxis Received From Day Of Start Of Hospitalization To Day After Admission Or Procedure” definition

      Please note the function “FromDayOfStartOfHospitalizationToDayAfterAdmission(Encounter, “Encounter Performed”) refers different start time below. The end point in this function refers inpatient admission (not hospitalization admission), where the QualifyingEncounter is called in the logic as “…starts during day of VTE."FromDayOfStartOfHospitalizationToDayAfterAdmission" ( QualifyingEncounter )”.

      Hello! 

      I have the following scenario and am hoping you can clarify a couple of things for us.

      A patient came through the ED- start Dt: 01/21/2025 to end Dt/Tm: 01/21/2025 2340

      Was an observation patient start Dt/Tm: 01/21/2025 2340 to end Dt/Tm 01/23/2025 1315

      Was made an IP start Dt/Tm: 01/23/2025 1315

      Had a procedure which included General Anesthesia on 01/24/2025 at 1445

      The patient did have intermittent pneumatic compression devices applied 01/24/2025 at 1445

      The guidance for this measure (v13) states for the Numerator

      "Inpatient hospitalizations for patients who received VTE prophylaxis:       

      • between the day of arrival and the day after hospital admission  
      • the day of or the day after surgery end date (for surgeries that end the day of or the day after hospital admission)"

      And the logic states:
      "Encounter with VTE Prophylaxis Received From Day of Start of Hospitalization To Day After Admission or Procedure"
      Is the "start of hospitalization" when this patient arrived at the ER and the Day after admission the day after the patient was made IP?

      In the scenario provided above, would this patient fall into the Numerator category?
