Thank you for your inquiry regarding CMS122v13 (2025 Performance Period). As specified in the guidance section of the header, "If the glycemic status assessment (HbA1c or GMI) is in the medical record, the test can be used to determine numerator compliance.". This measure requires documentation of an HbA1c or GMI lab test, identified by the "HbA1c Laboratory Test" (2.16.840.1.113883.3.464.1003.198.12.1013) value set or the "Glucose Management Indicator" LOINC code (97506-0), along with the test result and date. You will need documentation of the criteria mentioned above in the structured fields of the EHR for measure evaluation. If you have questions about reading the measure specification or understanding data requirements, you may refer to the "Guide for Reading eCQMs" for additional guidance. If you have questions regarding implementing the measure, you may refer to the "Implementation Checklist for eCQM Annual Update" for additional guidance. These resources can be found in the eCQI Resource Center: https://ecqi.healthit.gov/ep-ec?qt-tabs_ep=ecqm-resources&global_measure_group=eCQMs.

Hello, Glucose management indicator [GMI] was added as an acceptable method to assess numerator compliance in addition to the existing Hemoglobin A1c. What is the acceptable method of data collection? If a patient is not able to self-report their GMI from a CGM during the visit, how does the provider upload the GMI into the medical record? Does the patient provide a standardized CGM report summary, or does it require for the data to be imported from the CGM device/app into the medical record?
