How to account for contraindication for VTE that is correctly documented by physician and map correctly for ecqm credit.
If a patient has contraindications for SCDS, TEDS, and meds. (ie documented wounds that SCDS and TEDS would be contraindicated for. (meds d/c because surgery). Would this count towards numerator or denominator exclusion? I see in the guidance that "Reasons for no pharmacological and no mechanical VTE prophylaxis must be explicitly documented by the MD/APN/PA or pharmacist and linked with VTE prophylaxis. Ambulation alone is not a sufficient reason for not administering VTE prophylaxis. In order for ambulation/patient ambulating to be considered as an acceptable reason, there needs to be explicit documentation, e.g., "patient out of bed and ambulating in halls - no VTE prophylaxis needed." but doesnt specify whether to include in numerator and denominator or exclude denominator. It is also not listed in denominator exclusion. So where would a patient belong if fit the category of contraindicated and/ or the ambulation documentaiton above. I looked in the ecqm answer tool and did not find answer to this.
