Thank you for your inquiry regarding CMS156v12 (2024 Performance Period). Refills count as a separate prescription and contribute to the total days’ supply of the medication. As stated in the measure header "Definition" section, "an order is identified by either a prescription order or a prescription refill." Two separate medication orders (including refills) count toward meeting the numerator, provided the criteria (e.g., days’ supply or dose) are met.

Please review the measure specification to understand measure requirements: https://ecqi.healthit.gov/sites/default/files/ecqm/measures/CMS156v12.html. For additional guidance on how to read the eCQM specification, you may refer to the "Guide for Reading eCQMs" (https://ecqi.healthit.gov/sites/default/files/Guide-for-Reading-eCQMs-v9.pdf).

I have a question about Quality category's eCQM Measure 238 - Use of High Risk Meds in Older Adults.

      Question:

Is counting refills a requirement of the numerator of eCQM 238?

      Our initial thoughts:

Based on the example on pg 2 of the spec, the refill doesn't count as a 2nd med, but it does count in the total days that medication was prescribed for. Also, 2 orders of the same med would count as a second med if the row only has one medication.

The eCQM should count any high risk med in the row (including a second of the exact same med) as a 2nd medication.

Is the above correct and/or share details if it is not correct. Thank you!!
