      Thank you for inquiring about the Preventive Care and Screening: Screening for Depression and Follow-Up Plan eCQM. Please note that the measure’s Guidance section explicitly states the following:
      “Follow-Up Plan:
      The follow-up plan MUST still be provided for and discussed with the patient during the qualifying encounter used to evaluate the numerator.”
       
      Therefore, if your workflow only includes documentation of the referral to their PCP in the “After Visit Summary” and not an explicit discussion with the patient, it does not meet the measure's intent. Follow-up is expected to include active engagement with the patient about the plan, not simply the presence of documentation.

      For the purposes of satisfying the Follow-Up Plan component of the Depression Screening and Follow-Up Plan eCQM, we will be requiring specialist providers to refer patients with positive depression screens to a primary care provider. We will be using SNOMED 703978000 (Referral to primary care service [procedure]) to satisfy the numerator criteria. The workflow that will be associated with this code will be the provision of written instructions for the patient to schedule an appointment in primary care. These instructions will appear in the paperwork that is given to the patient upon checking out from their visit to the specialist (the “After Visit Summary”). Alternatively, the patient may access these instructions electronically via the patient portal. Please verify that this workflow is sufficient to support the use of SNOMED 703978000 for eCQM reporting and therefore satisfy the numerator criteria for the measure.
