      Thank you for inquiring about the Preventive Care and Screening: Body Mass Index (BMI) Screening and Follow-Up Plan eCQM. Regardless of whether a diagnosis indicative of an abnormal BMI or an abnormal BMI is documented, the follow-up intervention must be explicitly linked to the abnormal BMI using the appropriate value set codes (in “Overweight or Obese” or “Underweight”). Your organization’s specific documentation workflow may require the diagnosis, but the measure only requires linkage to abnormal findings (whether that is a BMI result or diagnosis).

      The Guidance section of the eCQM states the following:

      Follow-Up Plan Guidance:
      The documented follow-up plan must be based on the documented BMI, outside of normal parameters, example: "Patient referred to nutrition counseling for BMI above or below normal parameters."

      Good afternoon!

      For 2025 MVP

      I want to check and see if when we document a follow-up plan of documenting education, does that education have to be ordered under a diagnosis of underweight, overweight, or obese like it did in the past?
