      Thank you for your question regarding CMS138v13 (2025 Performance Period). This measure is based on the US Preventive Services Task Force (USPSTF) which references the US Food and Drug Administration definition of tobacco, which includes e-cigarettes. According to the 2021 USPSTF recommendation, e-cigarettes usually contain nicotine, which is the addictive ingredient in tobacco. Substances other than tobacco can also be used to smoke or vape.

      The "Tobacco Non User" (2.16.840.1.113883.3.526.3.1189) and "Tobacco User" (2.16.840.1.113883.3.526.3.1170) value sets include codes for e-cigarette use. We do not provide prescriptive mapping guidance on the value sets. If these value sets are not supported by your EHR, you may map to clinically equivalent codes. Please maintain documentation for any CMS audit.       Patients identified as using non-nicotine substances in e-cigartettes during screening--  are they considered tobacco users or non-tobacco users? If non-tobacco users, what snomed code should they be mapped to?
