Skilled nursing facilities (SNFs) and rehab facilities are not included in the "Discharge to Acute Care Facility" value set (OID 2.16.840.1.113883.3.117.1.7.1.87) and should not be excluded from the measure denominator.

      If pharmacological prophylaxis is not given due to a medical reason, then a medical reason for not administering VTE prophylaxis ( for both pharmacological and Mechanical) must be explicitly documented by MD/APN/PA or pharmacist within the qualifying timeframe for measurement to meet VTE-1. An order for SCD alone will not satisfy the numerator condition.

The initial population includes inpatient hospitalizations that end during the measurement period for patients age 18 and older and at least one opioid medication administration starts during the hospitalization outside of the operating room. This initial population applies to all inpatient hospitalization not just inpatient surgery patients.

      1. Safe Use of Opioids - Concurrent Prescribing CMS eCQM ID CMS506v6

      Is a skilled nursing facility or subacute rehab considered part of the denominator exclusion for " patients discharged to another inpatient care facility" or only patients transferred to another acute care hospital?

      2. Venous Thromboembolism Prophylaxis- CMS eCQM ID CMS108v12

      Is there a list of included "VTE prophylaxis" medications that would satisfy this measure? In particular- is Heparin, Enoxaparin, Apixaban, Clopidogrel, Aspirin 81mg twice daily OR Aspirin 325 once daily, and Warfarin included? Does provider documentation of a contraindication to medication prophylaxis and an order for sequential compression devices (SCD) qualify instead?

      3. Hospital Harm - Opioid-Related Adverse Events CMS eCQM ID CMS819v2

      Are all inpatients included in this measure or just inpatient surgery patients? I am not sure if this is relevant to our patient population as all of our surgery patients are outpatient only, However, we have non-surgical inpatient patients who receive opioids.
