      Thank you for your inquiry about CMS155v13 (performance period 2025). This measure includes multiple rates and stratifications, and it should be reported accordingly. The numerator criteria can be met through any of the following:
      1. Rate/Population 1: Patients who had a height, weight and body mass index (BMI) percentile recorded during the measurement period,
      2. Rate/Population 2: Patients who had counseling for nutrition during the measurement period, or
      3. Rate/Population 3: Patients who had counseling for physical activity during the measurement period

      The description of the measure is as follows: Percentage of patients 3-17 years of age who had an outpatient visit with a primary care physician (PCP) or obstetrician/gynecologist (OB/GYN) and who had evidence of the following during the measurement period. Do all three criteria need to be met to receive credit for this measure or do we get to select the subpopulation that we want to focus on? 

      • Percentage of patients with height, weight, and body mass index (BMI) percentile documentation -
      • Percentage of patients with counseling for nutrition
      • Percentage of patients with counseling for physical activity.
