Inquiry Regarding CMS69v3 Body Mass Index (BMI) Screening and Follow-Up

      Thank you for inquiring about Preventive Care and Screening: Body Mass Index (BMI) Screening and Follow-Up Plan eCQM. Please note that there are two pathways for meeting numerator criteria for the measure:
      i. BMI outside of normal parameters during the measurement period with a documented follow-up (explicitly referencing the abnormal BMI as the rationale) within the measurement period
      ii. Diagnosis of Overweight or Obese with an intervention ordered during the measurement period (either on the day that the diagnosis was first documented or any time before the Overweight or Obese diagnosis ends during the measurement period)

      The measure Guidance section explicitly states the following:
      "- If the documented BMI is outside of normal parameters, then a follow-up plan is documented during the encounter or during the measurement period.
      - If more than one BMI is reported during the measurement period, and any of the documented BMI assessments is outside of normal parameters, documentation of an appropriate follow-up plan will be used to determine if performance has been met…
      Follow-Up Plan Guidance:
      The documented follow-up plan must be based on the documented BMI, outside of normal parameters, example: "Patient referred to nutrition counseling for BMI above or below normal parameters." ”

      Therefore, with the example you provided, since the patient had a high BMI on 7/1, the patient will only meet numerator criteria if the high BMI intervention performed on 9/1 explicitly references the follow-up plan being related to the patient having a BMI outside of normal parameters (rather than another condition, or no rationale provided).

      1. Pt has visit with high BMI on 7/1
      2. Pt has visit where BMI goes slightly below the abnormal range on 8/1
      3. A valid high BMI intervention is performed 9/1
