      Thank you for your inquiry about CMS122v13 (2025 Performance Period). The intent of the measure is to look for a patient's most recent HbA1c level to be greater than 9.0% or is missing, or was not performed during the measurement period. At this time, this measure is not specified to capture and include those patients who have had a fructosamine test. The measure steward, the National Committee for Quality Assurance, will consider this update for a future iteration of this measure. Equivalent services can be mapped to the "HbA1c Laboratory Test" (2.16.840.1.113883.3.464.1003.198.12.1013) value set. If mapping is done, you should maintain documentation in case of a CMS audit.

    • Some patients require a test for fructosamine as they are unable to get a reliable blood sugar from an A1c check.

      Resurfacing this question that was originally asked in 2021 (CQM-5058). 

      The original solution stated "The intent of the measure is for a patient to meet numerator criteria if (1) the patient's most recent HbA1c level is greater than 9.0% or (2) the patient’s most recent HbA1c is missing, or (3) an HbA1c check was not performed during the measurement period. Unfortunately, a fructosamine test cannot be added to this measure because it does not meet the measure's intent."

      Now that this intent has adjusted for 2025 to include GMI, we wanted to inquire as to whether or not there is any discussion for future versions to include fructosamine tests?
