      Thank you for your inquiry regarding CMS819v3, Hospital Harm - Opioid-Related Adverse Events. As you noted, this measure uses the Global."HospitalizationWithObservation" function to determine the interval of the entire inpatient hospitalization encounter, which includes time in the ED and observation when the transition between these encounters and the inpatient admission are one hour or less. In order to assist you further, what time did the patient’s ED visit end?

      Patient arrived to ED at 1309, receiving Dilaudid 1mg IV at 1457 in the Emergency Room and then received Narcan 0.4mg IV at 2015, also while in the Emergency Room. Patient was admitted to Inpatient services at 0128. This occurrence didn't occur within 1-hour of inpatient admission. The measure description is causing confusion with the statement included about the following: Inpatient hospitalizations: Includes time in the emergency department and observation when the transition between these encounters (if they exist) and the inpatient encounter are within an hour or less of each other.

       

      Please offer assistance with this definition and provide an explanation of how we fell out of this measure, given that patient was admitted more than 3-hours post administration.
