      Thank you for your inquiry for CMS165v13 (2025 Performance Period). In this measure, blood pressure readings performed by the clinical staff (e.g., medical assistant, nurse, nurse practitioner), are qualifying documentation for evaluating numerator compliance. Per guidance in the measure specification, as long as the blood pressure readings are documented in the patient's medical record, as approved by the clinician, then those are acceptable for numerator evaluation. The intent of the guidance is to not include blood pressure readings that are read by or taken by patients.       I understand that blood pressures taken during a nurse visit no longer count towards this measure.  I had a provider ask if they could go in and addendum their visit note where they ordered the nurse visits to control blood pressure and enter the most recent blood pressure of the year. We just want to make sure this would not cause an issue if we were to be audited and that we have the proper documentation to support controlling a patients blood pressure. Thank you for your help
