PC-07 OB Complication - Day Of Logic For Procedure Captured Hysterectomy From Another Hospital

      Thank you for your question regarding CMS1028v3: Severe Obstetric Complications. From pilot testing during measure development, we observed that while procedure dates were available in the EHR, procedure start and end times were not consistently available for reporting. Therefore, "starts during day of" was incorporated into measure logic. We will continue to monitor measure performance and consult subject matter experts on the matter for future consideration.

    • Hospitals that transfer OB patients could have complications from the hospital they transferred the patient to

      We have a critical access hospital that has a procedure (hysterectomy) that is being counted as a complication at that hospital, even though the procedure occurred at the hospital the patient was transferred to on the same day of transfer.  Our EHR analyst noted this complication is being attributed to the transferring hospital due to the wording of "starts during day of" and nothing specific to the procedure having to occur during the actual encounter. Is this something that can be taken to CMS to address in future measure updates? For a small critical access hospital to be attributed a complication that occurred at another hospital is difficult for stakeholders to understand and with low volumes it makes their complication rate much higher than it should be.
