CMS128: Query on calculation of cumulative medication duration

      Thank you for your inquiry regarding CMS128v12 (2024 Performance Period) and CMS128v13 (2025 Performance Period). Based on the current eCQM logic for CMS 128, gaps in antidepressant medication are allowed when calculating the cumulative medication duration, as the measure permits up to 31 days of gaps within the 115-day period (Numerator 1) or 52 days within the 232-day period (Numerator 2). These gaps can occur either due to medication changes or refills. Therefore, there is no need to exclude medication gaps when calculating cumulative duration.       We have query on calculating medication duration for CMS 128: Anti-depressant Medication Management.

      The numerator clause definition mentions cumulative medication duration for anti depressant medication.
      However, in metadata (definition section) there is mention of The "continuous treatment" described in this measure allows for gaps in medication treatment does not align with logic of cumulative medication duration.

      Could you please let us know if we need to allow the gap in the medication while calculating the cumulative medication duration?

      If yes, could you update clause definition of cumulative medication duration so as to align with the definition of continuous treatment mentioned in the metadata.
