Thank you for your inquiry regarding CMS996v5: Appropriate Treatment for ST-Segment Elevation Myocardial Infarction (STEMI) Patients in the Emergency Department (ED). The measure captures three potential numerators for patients that arrive to the ED with a STEMI diagnoses: fibrinolytic therapy within 30 minutes, PCI procedure within 90 minutes, or discharge to acute care facility within 45 minutes. Per the measure logic, the patient would appropriately count under the second numerator, since they arrived to the ED with a STEMI diagnosis.

    • Patient pulling in as ED STEMI patient

When a STEMI patient is transferred to our hospital from another hospital's ED for PCI, our process is to have EMS stop in our ED (prior to going to the cath lab) for the patient to get registered and for the ED physician to make sure the patient remained stable during EMS transport. The ED performs a quick assessment and obtains a set of vital signs. If the patient is stable, the patient is taken directly to the cath lab, however they are initially registered as an ED patient. Is it appropriate that these patients are included in this STEMI measure?
