The specs for HH-02 indicate that any diabetes diagnosis before or during the encounter will qualify the patient for this measure. Perhaps limiting it to just a diagnosis present during the encounter or a maximum definitive previous time span up to the time of the encounter will ensure reporting is based on accurate and active coding seen throughout every encounter for the patient. Our system-wide EHR is shared among several hospitals and urgent cares and as a result we are seeing several encounters being pulled into the measure because there was incorrect coding or an errant diagnosis placed on a chart from a long ago encounter that identified them as incorrectly having diabetes. For example, we have a patient being included in the measure in September 2024 because there was a diabetes diagnosis in 2019. The patient has had 51 encounters since that initial visit and has no further diabetes diagnosis documentation. It's extremely time consuming to try to track down where the errant codes are coming from in the entire patient's history. A true diabetes diagnosis would be captured again during the current hospital admission the measure is reviewing.
