Please note that this is an episode of care measure, which means that you report on it for each non-telehealth encounter. In the example provided, there were two different encounters. Therefore, for the first (PCP) episode of care, the encounter would be in the numerator (due to a normal BP). The provided information made it unclear whether the follow-up was completed for the second (Ortho) episode of care where the BP was elevated. If appropriate follow-up were documented, then the second encounter would be in the numerator as well (due to elevated BP, but appropriate follow-up conducted). If follow-up were not documented during the Ortho encounter, then the encounter would be in the denominator only (due to elevated BP but no follow-up).

      Question is
      If a patient has 2 visits on same day at different facilities. ie with their PCP and then 2 hrs later at a specialty clinic like orthopedics. 
      During the PCP visit - the BP is controlled --> so the outcome would be met, controlled, scheduled fu for 12 weeks; Howeve

      However during an orthopedics visit on the same day, the BP is recorded as being elevated triggering counseling and 2-4 week follow up.

      Which outcome would should be used here- Met done? Met controlled?
