The narrative section of CMS50 (Closing the Referral Loop: Receipt of Specialist Report) states that "The clinician who refers the patient to another clinician is the clinician who should be held accountable for the performance of this measure."
As per the narrative guidance, the measure is expecting the first referral for a patient is considered for each clinician who orders a valid referral (First referral at a provider level). The measure's result for the patient should be attributed to each such provider.
Also, the expectation is that each such referral should have consult note tied to the referral in order to satisfy the numerator. If a first referral for one provider (Provider A) has a consult note tied and a first referral for another provider (Provider B) doesn't have a consult note tied then Numerator for Provider A will be 1 where as for Provider B will be 0 for the same patient.

However, the CQL specification of the measure CMS50v12 does not reflect the same logic as the narrative guidance. There is no mention of the referring provider for the first referral in the denominator or the numerator logic of the CQL.
This is discrepant from the narrative guidance and the guidance from CMS.
Is there a plan to revise the CQL logic to align it with the narrative guidance? If yes, is there an ETA?
