Thank you for inquiring about the Preventive Care and Screening: Screening for Depression and Follow-Up Plan eCQM. Please note that the measure guidance explicitly states, “This measure does not require documentation of a specific score, just whether results of the normalized and validated depression screening tool used are considered positive or negative. Each standardized screening tool provides guidance on whether a particular score is considered positive for depression.” Therefore, you would need to refer to PHQ-9’s specific guidance on interpreting the patient’s depression level. If a score (validated tool agnostic) shows no depression, that will map to “…negative (finding)”. If the score (validated tool agnostic) shows even mild depression, that would map to “positive (finding).” The degree of intervention opted by the clinician should be based on where the patient falls within the depression spectrum.

We have a client who would like to get an idea of their CMS2 performance. They have PHQ9 LOINC results and would like to map these LOINCS to the positive/negative screening codes matching this measure.

The idea is to map an adult PHQ9 with a result of 10 to the Adult Depression Screening Assessment valueset with a result matching the Depression Screening Positive Finding direct reference code.

There is some confusion regarding which PHQ9 values are considered "positive" for the purpose of this measure. Should PHQ9 results greater than 5 should be considered positive for the purpose of this measure? Or should only PHQ9 results greater than 9 be considered positive for this measure?
