To capture this occurrence, the discharge disposition needs to be documented as “Discharge to Acute Care Facility” in accordance with STEMI measure specifications and the Quality Data Model (QDM). If a patient if transferred from the ED to another unit (acute care) at the same facility - what is the disposition? We do not input disposition on the 'transfer/admit' from our ED. When the patient is truly discharged from the facility this is where disposition will come into play.
