      Thank you for your inquiry regarding CMS0996: Appropriate Treatment for ST-Segment Elevation Myocardial Infarction (STEMI) Patients in the Emergency Department (ED). Discharging the patient from the ED to receive acute care within the same facility will count toward the numerator as one of the possible numerator outcomes. To capture this occurrence, the discharge disposition needs to be documented as “Discharge to Acute Care Facility” in accordance with STEMI measure specifications and the Quality Data Model (QDM).       If a patient if transferred from the ED to another unit (acute care) at the same facility - what is the disposition? We do not input disposition on the 'transfer/admit' from our ED. When the patient is truly discharged from the facility this is where disposition will come into play.

      We are not sure how to code for the disposition from the ED to another unit within the same facility. Disposition for our facility is determined at actual discharge from the facility.
