The measure does not exclude cases where the STEMI develops during the ED encounter. The issue you have raised regarding the STEMI onset timing has been brought to the measure developer’s attention and the measure developer will consider refinements in a future update.

    • Patients can end up in numerator after arriving in ED without having a STEMI.

      How can a patient be correctly excluded from the measure given the 90 minute door-to-balloon time or transfer to acute care 45 minutes if the STEMI occurs in the ED but for example, 2 hours after arrival. It does not seem that there is a correct exclusion that this patient would fall into.
