      Thank you for your question regarding CMS832v2, Hospital Harm – Acute Kidney Injury.

      1. For creatinine values outside the limit of detection, the EHR can record character value results. For example, the character value could be recorded as “<0.9 mg/dL” or “not detected”. Values outside of the limit of detection should be accurately mapped to a value (i.e., decimal) for purposes of reporting. When mapping a character value, please make sure to document your process.

      2. Specific to the timing of kidney dialysis, the logic for definition ‘Encounter with Kidney Dialysis Started 48 Hours or Less After Arrival’ is correct, as the logic assesses for a dialysis procedure code during the start of the qualifying encounter, up to and including 48 hours after encounter start. Your interpretation of the logic is correct for definition ‘Encounter with Kidney Dialysis Started More Than 48 Hours After Arrival’. The measure intent differs from the CQL logic. The intent is to evaluate for a dialysis procedure code that begins 48 hours after the start of the qualifying encounter through the end of the encounter. We will update the logic in a future Annual Update cycle.

      eGFR question:
      The eGFR values require creatine results to calculate the equation, should we accept creatine results with greater-than or less-than symbols (ex. >0.9)? If yes, how should we handle these results to get an accurate value for eGFR?

      Hospital Dialysis 48 hours or less after arrival, and Hospital Dialysis more than 48 hours after arrival question:
      For denominator exclusions the definition states "Encounter with Kidney Dialysis Started {}48 hours or Less After Arrival" and the CQL checks for a dialysis procedure from the start of hospitalization + 48 hours. The numerator definition states "Encounter with Kidney Dialysis Started More Than 48 hours after arrival" but the CQL checks +48 hours after hospitalization. Unless we are interpreting this incorrectly, this does not seem to align with the numerator definition as both populations are looking at the 48-hour mark. Can you clarify this for us? If a patient has dialysis exactly 48 hours after hospitalization, should the patient qualify for denominator exclusions or numerator?
