      Cath lab is utilizing a third-party vendor that isn't integrated with the main EHR system, and the data cannot be extracted for the CMS eCQM submission. Any workaround will create the disruption to the clinician's workflow and duplication of documentation.

      Cath lab is utilizing a third-party vendor that isn't integrated with the main EHR system, and the data cannot be extracted for the CMS eCQM submission. Any workaround will create the disruption to the clinician's workflow and duplication of documentation.

      Currently, our hospitals document Cath's lab work using a third-party vendor tool, which cannot be integrated with the main hospital EHR system; as a result, certain required data elements (such as PCI documentation) are missing in EHR system.

       A few questions:
      1. If a clinician documents the PCI date/time in McKesson, a third-party vendor tool, is it appropriate and acceptable for non-clinical staff to review the McKesson tool and abstract the PCI date/time into the EHR record to ensure the documentation is captured for the eCQM?
      2. If yes, can this data element be abstracted into the patient record retrospectively after the patient's discharge, or does it have to be documented before the discharge?
