      Thank you for your question regarding CMS506v6: Safe Use of Opioids - Concurrent Prescribing. For your first question, if a patient has an order for palliative care consult, and that order is manually discontinued prior to patient discharge, the patient would not be excluded because the order was not actually ordered.

      For your second question, if an order was placed but the patient refused or the consult did not occur, this order would still count towards the denominator exclusion. An order for a consult for palliative or hospice care would meet the requirements for the denominator exclusion, regardless of the outcome.

      1. We would like clarification on palliative care exclusions, if patients have an order for palliative care consult, and that order gets manually discontinued few days later prior to patient discharge. Will that patient be automatically excluded from the measure.

      2. Also, if an order was placed but the patient refused or consult didn't occur, and regardless of the reason the consult was ordered, will that also place the patient automatically in the exclusion criteria
